She is survived by: her children, Debbie, Bryan Dank, Jody and Kenny Barnet; her grandchildren, Tyler Dank, Maya Dank, Jesse Barnet and Ally Barnet; her parent Barbara Forman (Kaplan); her siblings, Milt Lewis, Ed Forman and Jan Swanberg; her nieces, Leslie and Michele (Matt); her nephew Ben (Emma); and also Katie.
