He was predeceased by: his parents, John Tremblay and Alice Tremblay. He is survived by: his son Dominic; and his sisters, Lynda (Mickey Berkowitz) of Newburyport and Karin Tremblay of Newburyport.

In lieu of flowers, donations can be made in Jack's name to the American Cancer Association or One Tree Planted.
