He was predeceased by: his parents, Henry Levine and Kate Levine (Koufax); and his wife Caroline Sheehan Levine. He is survived by: his siblings, Stephen Levine (Joan) and Elizabeth Levine Levy (Mark); his nieces, Jessica Backman-Levy (Paul), Sarah Levine Meyer (David), Rachel Levy (Sixto Franco) and Emily Levine (Jake Javitch); and his great-nieces and great-nephews, Spencer Meyer, Chase Meyer, Odessa Franco-Levy, Poppy Javitch, Remy Javitch and Eliana Backman-Levy.
