He was predeceased by: his parent John Chwalek. He is survived by: his parent Anna Chwalek (Chrzan); and his brother-in-law Edward G. Perlak of Ludlow, MA. He is also survived by several nieces, nephews, grandnieces and grandnephews.

At Fr. Chwalek's request, in lieu of flowers, memorial donations may be made to St. Stanislaus School, 108 Summer St. Adams, MA 01220.
