He was predeceased by: his grandson Derek Fortmayer; his sister Geraldine Gautreaux; and his parents, Gladys Pertuit Fortmayer and Henry Fortmayer. He is survived by: his wife Evelyn Palmer Fortmayer; his children, Jan Fortmayer Dybul (Gary) and Ken Fortmayer (Cheryl); his grandchild Aubrey Fortmayer; and his siblings, Henry "Butch" Fortmayer (Faye) and Sharon Fortmayer. He is also survived by other loving relatives and friends.
