He was predeceased by: his parents, Woodrow P. Van Slyke and Pearl Loftin. He is survived by: his sister Ann Harrell (Mahlon) of Westlake; his niece Mandy Sanders (BJ) of Lake Charles; his nephew John Harrell (Katie) of Butler; his great-nephews, Noah, Chamis and Larry; and his great-niece Lona.
