He was predeceased by: his parents, Clarence Cook and Dora Lee Cook; his grandson Killian Lenard; his sister Patricia Ann; and his brothers-in-law, Roy Parmentier and David McDonald.
He is survived by: his children, Clarence Cook (Carly), Krystal Beall (Edward), Robin Cook, Debra Mobley (Doyle) and Pamela Lenard; his stepchildren, Katherine Stutts (Donnie) and Kenneth Stanford; and his sisters, Donna McDonald, Barbara Fuller and Grace Cook. He is also survived by his numerous grandchildren and great grandchildren.
