C. Chism (Teresa), Kenneth Chism (Carol), Jessie Chism (Rosemary), Betty Floyd (William), Jackie Chism (Judy), Mitch Chism (Tammy) and Bill Chism (Brenda); and his sister-in-law Pat Chism.
In lieu of flowers, memorials may be made to First West or to the Alzheimer's Association.
