He is survived by: his wife Gwendolyn Smith; his children, Michelle Underwood (David), Fordie Cook, III and Chanah Crow (Steve); his grandchildren, Elizabeth, Gavin, Kereston, Fordie IV, Hillary, Jonah, Shooter and Mary Annah; and his brother Ted Smith. He is also survived by numerous nieces and nephews.
