He was predeceased by: his parents, Garland Lee and Zella Murray Forman; his former wife Velma Keopp Forman; and his wife Carmen Forman. He is survived by: his children, Garland Ray Forman, Jr. (Debbie) of Bayou Chicot, Patricia Forman Bagwell (Lloyd) of Pineville and Rebecca Forman Sawyer (Tim) of Pearl River; and his stepdaughters, Ramona Herman (Dan) of Vinton, Patty Minter (Shaun) of Houma, Penny Landry of Houma and Diana Guidroz (David) of Houma.
Visitation was held on Thursday, March 13th 2025 from 4:00 PM to 9:00 PM and on Friday, March 14th 2025 from 11:00 AM to 1:00 PM at the Calvary Baptist Church - Bayou Chicot (1059 Calvary Baptist Rd, Ville Platte, LA 70586). A funeral service was held on Friday, March 14th 2025 from 1:00 PM to 1:30 PM at the same location. A burial was held on Friday, March 14th 2025 from 1:30 PM to 2:00 PM at the Vandenburg Cemetery.
