He was predeceased by: his parents, Mary Mayzak and Charles Mayzak; his brother Peter Mayzak; and his wife Sharon (Van Pelt). He is survived by: his siblings, Trena Scheffler, Wilimina Antochi (Willy) and CJ Mayzak; his former wife Marilyn LaGrange; his daughters, Mary Tidwell (Mayzak) and Veronica Mayzak; and his grandchildren, Cecelia Tidwell, Addison Tidwell, James Miller, Amanda Ohler, Kayla Flood, Kuira Flood and Kristy Miller.
