He is survived by: his children, Jason Middlebrook, Austin Craig, Jacob Middlebrook (Rebecca Rutzler) and Taylor Middlebrook (Matthew Magee); his siblings, Vicki Adams (Kevin Adams), Debi Boyt, Wanda, Eric Middlebrook (Melissa), Autumn Black (Brad) and Davi Campbell; his stepsister Monica; and his grandchildren, Stormi Middlebrook, Payton Middlebrook, Michelle Rutzler and Isaac Middlebrook.
A memorial service will be held on Monday, January 20th 2025 from 12:00 PM to 2:00 PM at the Aulds Funeral Home (7849 E Kings Hwy, Shreveport, LA 71115).
