He was predeceased by: his grandparents, Earl B., Ernestine Gordy, John and Roberta Salisbury. He is survived by: his children, Theresa M. Patingo (Kelly), Kinsley, Karsyn, Alyssa S. Prince (Mike), Anakin, Kennedy, Nickolas S. Salisbury and Justin M. Salisbury; his parents, Gertrude G. Salisbury and John J. Salisbury, Jr.; and his sisters, Stacey S. Vollenweider (Robert) and Jennifer L. Salisbury (Laron). He is also survived by nieces, nephews, great nieces, and nephews.

In lieu of flowers, the family is asking that contributions be made to the American Diabetes Association.
