He is survived by: his parents, Ursula Faye Hawkins and Ambrose Forman, Sr. of Rayne, LA; his fiancée Wendy Ann Harrison; his children, Keenan Christo Forman of St. Martinville, LA, Ambra Shae, Alaiya, Blaze Forman of Rayne, LA and Tamrie Comeaux Forman of Rayne, LA; his siblings, Marlo Forman of Rayne, LA, Therion Wilridge of Albany, Amber Wilridge of Albany, Rene and Gatha Delco; his grandmother Jane Lois Harmon of Rayne, LA; and his godparents, Margaret Plummer of Rayne, LA and Kenneth Senegal of Missouri City, TX. He is also survived by godchildren, nieces, nephews, relatives, and friends.
"Bros" was preceded in death by his maternal grandfather, Lester Harmon, Sr.; paternal grandparents, Lillie Mae Domingue Forman and Willis Forman, Sr.; and cousin, Mitchell DeVaughn Evans. Friday, September 11th, 2020: Public viewing at the Sanctuary of Worship from 6pm-9pm Saturday, September 12th, 2020: Public viewing at the Sanctuary of Worship from 8am-10:30am Due to COVID-19 regulations, the funeral will be private and for immediate family only. The family has set up a viewing projector at 112 Pine Street, in Rayne, LA, so that friends and family may social distance while watching the live stream of the service.
