He was predeceased by: his parents, Millard Hook and Ona Partin Hook; his brothers, Anthony Hook, Erwin Dole Shelton and Gary Shelton; and his pet Samantha.
He is survived by: his wife Amanda Hook; his daughters, Cynthia Hook (Jason Millen) and Alexandra Hook; his stepson Christopher Gagnard (Krista Bickley); his brother John Shelton; his grandchildren, Logan Uhrbach and Kenzie Gallion; and his nephew David Hook.
