He was predeceased by: his parents, Bennie Joseph Fazio, Sr. and Josephine Nasella Fazio of Alexandria, LA.. He is survived by: his wife Helen Crooks Fazio; his sons, Mark Fazio, Kris Fazio of Tampa, FL, Todd Fazio, Sheila Fazio of Rosepine, LA, Bart Fazio and Amy Fazio of Rosepine, LA; his grandchildren, Trent Fazio of Philadelphia, PA, Grant Fazio of Orlando, FL, Tate Fazio, Macy Fazio of Rosepine, LA, Luca, Lilah and Lia Fazio of Rosepine, LA; and his aunt Joan Mahfouz of Alexandria, LA. He is also survived by numerous cousins, nieces, nephews and a host of family and friends.
