He was predeceased by: his parents, Fordie Nelson Cook Sr. and Ruby Claire James Cook; and his sister Sanda Custer. He is survived by: his close friend Nancy Colquette of Mer Rouge, Louisiana; his children, Fordie Nelson "Trey" Cook III of Beekman, Louisiana and Chanah; his son-in-law Steve Crow of Monroe, Louisiana; his grandchildren, Fordie Nelson Cook IV of Beekman, Louisana, Kereston Crow of Monroe, Louisiana and Shooter Kane Cook of Beekman, Louisiana; and his stepgrandsons, Gavin Pollock of Beekman, Louisiana and Jonah Gorman of Beekman, Louisiana. He is also survived by two great grandchildren.
