He is survived by: his wife Kathleen Smith; his children, Rhonda Hendrix (Tom, Jr.), Christine Allen (Brian) and Nicky "Neal" O'Neal Smith, Jr.; his granddaughter Dina Hendrix Mayhall (Sawyer); his children, Bradley Allen (Carrie), Ashley Smith Wagner (Jacob), Taylor Allen (Chapman Bazer) and Hannah Smith; his great grandchild Rachel Allen; his grandchildren, Gabriel Allen, Tilly Mayhall and Tristin Mayhall; and his siblings, Ronnie Victor Smith and Debbie Cox. He is also survived by nieces and nephews.
The family request donations in his memory to be made to Start Baptist Church Children and Youth Ministry.
