He is survived by: his wife Mary Ann Forsyth (Millet); his children, Theresa Louque (Steven), Tammy Jacob (Byron), Tina Abadie and Leonard Forsyth (Brandy); his grandchildren, Jordan Duhon (Kelcie), Cody Duhon, Emily Jacob, Justin Jacob, Joshua Abadie, Olivia Forsyth and Mia Forsyth; his great grandchildren, Truett, Hollis, Quincy Duhon and Locklen Jacob; his sisters, Nancy Lasseigne and Penny Zeringue (Jim); and his siblings-in-law, Anna Forsyth and Wesley Hurley.
