She was predeceased by: her husband Norman "Craig" Forrest; her father W. C. Cook; and her sister-in-law Kathy Dale Forrest. She is survived by: her children, Trent Forrest (Macy) and Traci Forrest Jennings (Ryan); her grandchildren, Connor, Kale Forrest, Parker and Peyton Jennings; her mother Elaine Cook; and her siblings, Tam Brooks (Luke), Brad Cook (Leslie) and Randy Cook (Dawn). She is also survived by numerous nieces and nephews.
