He was predeceased by: his parents, Robert "Dale" Forrest and Catherine "Joyce" Blades Forrest; and his sister Kathy Dale Forrest (Pam). He is survived by: his wife Pam Cook Forrest; his children, Trent Forrest (Macy) and Traci Forrest Jennings (Ryan); and his grandchildren, Connor, Kale Forrest, Parker and Peyton Jennings.
In lieu of flowers, the family has requested that donations be made to Spring Creek Baptist Church or the Richard Murphy Hospice House.
