He is survived by: his grandmother Carol Leblanc (David Leblanc, Sr.); his girlfriend Abigail Johnson; his children, Annalee Derise and Ayden Johnson; his aunts and uncles, Morgan, Shonell LeBlanc, Herbert Derise, Jr. and David "D. J." Leblanc, Jr. (Tiffany); and his cousins, Alyssa, Trey Derise and Trevor Nelson.
