He is survived by: his son Chad Michael Hebert, Jr.; his father Thomas Curtis Hebert Sr.; his brother Thomas Curtis Hebert Jr. (Tracy); his former wife Gloria Breaux (Mitchell); his godson Trent Hebert; his nephew Gage Hebert; and his niece Jaden Hebert.
