He is survived by: his children, Stephanie Perkins Shealy (Jeremy) and Matthew Perkins; his grandchild Cannon Shealy; his brother Jay Perkins; his sisters-in-law, Laura Perkins and Retha Perkins; his cousin John Thomas "Tommy" Accomando, Jr.; his nephew Michael Jones; and his nieces, Abby Rainville and Ashley Perkins.
