He was predeceased by: his parents, Charlie Ballard and Eunice Ballard; and his siblings, Lucille Green, Lillie David, O. C. Ballard and Ruben Ballard.
He is survived by: his wife Kennie Perry Ballard of Forest Hill; his children, Curtis Ballard of Duson, Randy Ballard (Susan) of Oakdale and Rhonda Bailey (Paul) of Forest Hill; and his sister Trevia Clark of Livingston, Texas. He is also survived by five grandchildren; and ten great grandchildren.
