He was predeceased by: his wife Jacquelyn Ferchaud Levy; his parents, Leo Felix Levy, Sr. and Ruby Lefort Levy; his brother Arthur James Levy; his nephews, Jimmy Dawson and Arthur Baker; his parents-in-law, Beverly Braud Ferchaud and John Alex Ferchaud, Sr.; and his brother-in-law John Alex Ferchaud, Jr. (Lois) of Vacherie.

He is survived by: his children, Leo Felix Levy, III (Felisha) of Geismar, Ashley Tremayne Levy (Karmyn) of Geismar and Lan Levy-Jarvis (Ian) of Prairieville, Louisiana; his grandchildren, YatzMyne Levy, Ashton Levy, Eryn B. Levy, Aaron R. Levy, Alayna Ellis, Aspen Levy, Taylor Jarvis, Tyler Jarvis, Ashley Levy, II, Logan Jarvis and Kariel Jarvis; his siblings, Lydia Levy- of Dixon, Louie Ann Levy Hall (Stephen) of Donaldsonville and Nadine Levy Stewart (Ronald) of Vallejo, California; his siblings-in-law, Rodger Lester Ferchaud (Stephanie) of Saint James, Louisiana, Elaine Ferchaud Grant (Alfred, Jr.) of Saint James and Dr. Vanessa A. Ferchaud of Baton Rouge, Louisiana; his godchildren, Yolanda Levy and Ibiomi Pye; and his friends, James "Tom" Smith and Warren Thomas. He is also survived by other nieces, nephews other relatives and friends.
