He was predeceased by: his parents, Captain Smith Blake and Hazel Wallett Blake; and his sister Judy Rice. He is survived by: his wife Jackie Blake; his children, Chasaty Blake and David Horn (Eve); his grandchildren, Devin Primm, Victoria Primm and Edward Horn; and his siblings, Lurlene Dowden, Linda Jones, Bettie Campbell, Rebecca Blake and Shelia Thomisee.
