He is survived by: his children, Doug Middleton (Kim), Alan Middleton (Cindy), Cindy Avery, Stephanie Eastham (Alan) and Tammie Middleton; his sisters, Kathy Miller (Ray) and Sally Fletcher; his grandchildren, Kelli McManus (Brian), Jennifer Tyson (Brent), Colin Middleton (Alisha), Shane Middleton (Mallory), Caitlin Middleton, Melissa Matapi (Eliceo) and Emily Eastham; and his great grandchildren, Drew, Lainey, Peyton, Micah, Jane, Ashlyn, Stella, Liam and Austyn.
