Cazort IV (Juliana Goodwin); her granddaughter Isabella Caroline Cazort; her daughter Amye Cazort Martin (Louis); her brother David Max Fonda (Hedy Fonda); her nieces, Cheryle Lynn Breaux and Lisa Acklen; and her nephew David Max Fonda, III. She is also survived by her children and beloved grand-daughter.
