He was predeceased by: his wife Carolyn Elaine Scanlan. He is survived by: his friend Larry Johnson; his children, Amy Polk Baker (Ty Baker), Paul Polk (Marcia Polk) and Jerry Polk Jr. (Traci Polk); and his grandchildren, Shaydlin Baker, Isabella Baker, Alexandra Polk, Zachery Polk, Parker Polk and Jude Polk.
