She was predeceased by: her former husband Benjamin Bruce "Bubba" Mayes Jr.; her father Samuel C Micale; and her sisters, Cindy Salvatore and Debra Micale McWhirt. She is survived by: her sons, Notley Mark Lewis II (Sarah) of Orlando, FL and James Robert "J. R." Lewis (Amber) of Naples, FL; her grandchildren, Paulyanna Lewis, Notley Mark "Bubba" Lewis III and Kassidy Sullivan; and her mother Verla Mae Burleson Micale.
