He is survived by: his children, Shannon Keith Middleton (Jewel) of Bledsoe, KY, Phillip Edward Middleton (Loretta) of Bledsoe, KY, Harold Turner (Wilma) of Smilax, KY, Anita Sue McFarland of Cookville, TN, Elizabeth Middleton (Jimmy Lynn Adams) of Yeaddiss, KY, Jessie Ray Witt of Lexington, KY and Dustin "Dooter" Lewis of Yeaddiss, KY; his siblings, Rick Middleton (Ann) of Atlanta, GA, Hank Middleton (Nancy) of London, KY, Conda Griffith, Violet Duquet (Art) and Kathy Lee Hedrick (Stewart); his close friends, Bill Melton, Billy Ray Lewis, Jim Huff, Billy Shepherd and Johnny Gross; and his great grandchild Hunter "Rip" Lewis. He is also survived by thirteen grandchildren, forty four great-grandchildren and six great-great grandchildren.
