She was predeceased by: her parents, Edward Burgess and Izetta Bays Burgess; her husband Carl Mayo Cook; her son James Burgess Cooke; her granddaughter Sarah Ruth Prince; and her siblings, Arnold, Chester, Raymond, Ralph, Hubert, Clyde, James, Marjorie Craig and Garnet Ortega.
She is survived by: her daughter Karla Jean Cooke Prince (Mike Ellis) of Flatwoods, KY; her grandchildren, Charles Douglas Prince (Michael Moss) of Huntington, WV and Rebecca June Norment (Breck Norment) of Lexington; her great grandchildren, Liam and Caroline Ruth Norment; and her children, Annie Richardson, Matthew, Josh Bowling and Bea Smith (Andy).
