He was predeceased by: his parents, Arthur Lewis and Madge Moore Lewis; his siblings, Herman Lewis, Jerry Dale Lewis, Douglas Wayne Lewis, Darlene DeWitt, Terri Lynn Lewis and Virginia Mae Moore; and his mother-in-law Corda Foster.
He is survived by: his wife Sharon Foster Lewis; his siblings, John Paul Lewis (Glenna) of Morehead, Dennis Lewis of Clearfield, Barb Kegley (Bobby) of Morehead, Shirley Shay of Clearfield, Judy Cassara (Chuck) of Morehead and Sheila Lewis of Clearfield; and his niece Tasha McCarty (Kevin) of Morehead. He is also survived by nieces and nephews; extended family; and friends.
