She was predeceased by: her grandfathers, Wince Sargent and Doug Forman Sr.. She is survived by: her parents, Avelene Gay Sargent Wireman of West Liberty, KY and Douglas G. Forman Jr. of West Liberty, KY.; her children, Bailey Jade Ralls of Mt. Sterling, KY, Jacee Madison Lewis of West Liberty, KY and Jaxon Wade Hensley of West Liberty, KY.; her stepbrother Brandon Wireman of Jeffersonville, KY; her half-brother Shawn Forman of Morehead, KY.; and her stepsisters, Kayla Marie Wireman of West Liberty, KY and Heather Craft of Texas.
