He was predeceased by: his brother Ray. He is survived by: his significant other Susan; his son Jeremiah Cooks; his grandson Lucas; his siblings, Mary Odorowski (Gary), Theresa McKnight (Al), Cathy Czyzniewski (Dan), Pamela Beatty and Robert Cooks (Annette); and his cousin James Wagner (Lauren). He is also survived by numerous relatives and friends.
