He was predeceased by: his wife Geraldine Gollihue Church; and his brothers, Jim Church and Charles Church. He is survived by: his children, Johnny C. Church (Janet) of Willard, Debbie Porter (James Lee) of Webbville and Rhonda Colley (Michael) of Denton; and his siblings, Joe Church of Willard, Eddie Church of Lawrence County, Luke Church of Carter County and Thella Allen of Denton. He is also survived by five grandchildren; nine great-grandchildren and three great-great-grandchildren.
