He was predeceased by: his parents, Harold Stevenson Sullivan and Johnnie Maud Sullivan (Tanner); and his siblings, Mary Louise Sullivan and John Sullivan. He is survived by: his children, Tana Cooper (Steve) of Barlow, KY, Kimberly Wood (Jeff) of Dallas, TX, Larrah Workman (Jason) of Hickory, KY, Mike Sullivan (Monica) of Syracuse, UT and Tom Sullivan (Courtney) of Lebanon, KY; and his siblings, Shirley Griffith of Wickliffe, KY and Joe Sullivan of Sarasota, FL. He is also survived by several grandchildren and great grandchildren.
