He is survived by: his son Jeremy Polivick (Michelle) of Grovetown, Georgia; his siblings, Debby Watson of Clinton, Mary Louise Hoskins of Wickliffe and Gary Polivick of Wickliffe; his sisters-in-law, Jackie Polivick of Paducah and Bettye Polivick of Henderson, Kentucky; and his grandchildren, Melanie Knighten of Wickliffe, Jaren of Grovetown, Georgia and Jaden Polivick of Grovetown, Georgia. He is also survived by several nieces and nephews.
