He is survived by: his wife Glenda Day Cook; his children, Shawn Cook of Whitesburg and Kayla Kolb (Carter) of St. Petersburg, Florida; his brothers, Gary Cook of Isom and Tim Cook of Hindman; his grandchildren, Jacob (Sydney), Taylor and Finley; and his great grandchild Adrianne.
