She was predeceased by: her parents, Clyde Haynes and Mallie Johnson; and her husband Jimmy McIntosh. She is survived by: her children, Dena K. Collins of Hazard, Jammie Lynn Baker (Gregory S. Baker) of Thornton and Thomas Scott Collins (Jennifer Collins) of Ermine; her grandchildren, Kyle, Kierra Collins, Christian Collins, Trennity, Cameron Ritchardson, Pamela Collins and Jacob Elija Baker; and her great grandchildren, Emerald Ritchardson, Malaya Collins and Millie Collins.
