He is survived by: his former wife Pamela K. Haynes; his wife Pam McIntosh of Ermine; his children, Thomas Collins (Jennifer) of Ermine, Dena Collins of Whitesburg and Jammie Baker (Shane) of Thornton, Kentucky.; his grandchildren, Kyle Collins (Kierra), Christian Collins, Jacob Baker, Trennity Baker (Cameron Richardson) and Pamela Collins; and his great grandchildren, Emerald Richardson and Malaya Collins.
