He was predeceased by: his father Ancle Dale Perkins; and his grandparents, Orville, Delsa Caskey, Ventus and Nina Perkins.
He is survived by: his mother Kathleen Caskey Perkins of Kettering, OH; his sister Holly Perkins of West Liberty, KY; his nephew Nathan Perkins; his great-nephew Dalton Gebele; and his aunt Fern Carroll (Jerry) of Versailles, KY.
