He was predeceased by: his siblings, Theodore, Anthony, Leon Chlapowski, Pauline Paglione and Patricia Johnson; and his parents, Chester and Agnes Chlapowski (Delman).

He is survived by: his wife Jean C. Chlapowski (Cassidy); his daughter Susan A. Chlapowski (Aaron Perna) of Worcester; his stepgrandchildren, Chelsea, Taylor and Hannah; and his sister Eleanor Damian of Texas. He is also survived by nephews and nieces.

Donations in his name may be made to the Parkinson's Foundation, 200 Southeast First St., Suite 800, Miami, FL 33131.
