He was predeceased by: his wife Debbie Smith; and his parents, Everett Earl and Jean Fay Smith. He is survived by: his children, Bridget Linton (Matthew), Brandi Payton (Mike) and Shawn Smith; his siblings, Charlotte Scrogham (Gary), Karen Byers (David) and Glenn Smith; and his grandchildren, Cortney, Chloe, Cameron, Lillian, Brandon, Ryan, Carter and Makenzie.

Memorial gifts are suggested to the Kentucky Lions Eye Foundation or the American Cancer Society.
