He was predeceased by: his grandparents, Tommy, Evelyn Boaz and Bill Flaherty. He is survived by: his parents, Dennis Boaz and Kelli Ann Flaherty Boaz; his wife Crystal Anne Miller Boaz; his daughter Everleigh Anne Boaz; his sisters, Debbie Boaz and Florence; and his grandmother Lois Flaherty. He is also survived by nieces and nephews.
