He is survived by: his wife Susan Frazier of Radcliff, KY; his children, Philip Frazier Jr. (Elisabeth) of Texas, Michael Randlett (Kay) of Vine Grove, KY and Laura Whaley (William) of Florida; his brother David Frazier of Oklahoma; his grandchildren, Gavin Frazier, Brayden Frazier, Jenna Whaley, Bryce Whaley and Brock Randlett; his great grandchild Audi; and his sister-in-law Carol Freeburn (Chuck).
