She was predeceased by: her father Cleve Moncier; her husband Prentice Lee Forsythe Sr.; her siblings, Dewaine Moncier and Evelyn Knight; and her granddaughter Lana Lee Forsythe.
She is survived by: her children, Sandra Hunt, Sharon Broussard, Lee Forsythe Jr. (Bonnie), Lynn Forsythe (Karen) and Jeana Duvall (Alvin); her niece Dana Dillon; and her nephew Randall Castellaw. She is also survived by 14 grandchildren; 22 great grandchildren and 5 great-great grandchild.
In lieu of flowers donations may be made to Kentucky Humane Society.
