She is survived by: her parents, Brent Applegate and Carol Gasnik Applegate; her daughters, Breawna Smith, Raegan Clemmons and Brittany Clemmons of Lewis County; her husband Anthony Clemmons of Berea, KY; and her siblings, Nicole Sexton (Gordon) of Middletown, OH and Matthew Applegate of Lewis County. She is also survived by several nieces & nephews.
