He was predeceased by: his wife Ada May Lee Stevens; and his brother Daniel Stevens. He is survived by: his sons, David M. Stevens Jr. (Tammy) and Brian Stevens (Cheryl); his siblings, Sandy Hickman (Larry) of Louisville and Jimmy Stevens of Taylorsville; his grandchildren, Evan McKinney (Taylor), Jacob Stevens, Courtney Bentley (Dustin), Caitlyn Stevens and Chelsea Stevens; and his great grandchildren, Laiken and Parker McKinney.
